FAMILY MEETING PLANNER

Give the conversation a shared shape.

A family meeting works best when there is room for your relative’s wishes, clear questions, and a realistic division of practical tasks.

Before the conversation

Ask permission

Agree who should join and what information your relative is comfortable sharing.

Keep the scope clear

Choose a practical topic you can discuss. Gather clinical questions for a qualified healthcare professional.

Make room to listen

Invite preferences before assigning tasks. Leave space for uncertainty and disagreement.

WORKING SHEET

A conversation, then a checkpoint.

Notes stay only in this tab’s memory. Nothing here is sent or saved. Printing uses your browser’s print dialog; avoid entering sensitive personal or medical information.

If family members see things differently

Write down the different questions before trying to settle them. Check what is a confirmed fact, what is a preference, and what needs more information. A task can be assigned without pretending every larger decision is resolved.

Give a family update a simple shape: what was discussed, what was agreed, what is still open, and who follows up. Share only what your relative has agreed may be shared.
