A family plan can stay in your tab
The conversation-agenda tool handles selected task topics and the audience for a discussion. The pathway comparison handles the topics you select. The family meeting sheet handles the notes you type about wishes, open questions, tasks, and responsibilities. These tools use the current page’s memory: the site does not send their inputs to us or store them in cookies, local storage, or a server database. A fresh page load resets the tools; browser history restoration, autofill, and files you create are controlled by your browser, not a storage feature of this site.
Choosing to print an agenda or meeting sheet opens your browser’s print function. A printout or file you choose to create is under your control, not collected by the tool. Avoid putting sensitive medical details or information about someone else in the working sheet. Resource searches and filters also operate in the browser and are not submitted to us.
What you choose to send, and what a request carries
Information leaves the planning page only through the separate contact or subscription form when you submit it. We receive the following categories:
- Contact messages: full name, email address, and the message you enter. We ask you not to include medical records or unnecessary sensitive information. If you include personal details in a message, those details become part of the submission.
- Caregiver Notes subscription requests: first and last name, email, Date of Birth for the adult-age check, and a phone number if supplied. A phone number is required only if you choose SMS; otherwise it may be left blank.
- Permission evidence: the email, SMS, and legal-acknowledgment checkbox states; the exact consent wording and its revision version; the submission timestamp; the website hostname; and, for subscriptions, the page URL.
- Ordinary technical request data: hosting and form-processing infrastructure may receive an IP address, browser or device information, requested URL, request time, network headers, response status, and diagnostic or security logs. The site does not run a browser-based IP lookup or add an IP field to form payloads.
- Security verification: if a form offers reCAPTCHA, its response token is sent with the form to the verification gateway. The gateway checks it with Google before forwarding the submission. The token is used for that security check, not added to the newsletter or contact record.
- Operational correspondence: replies, delivery information made available by communication providers, privacy requests, and opt-out or suppression records needed to administer your choices.
We do not ask for medical histories or clinical assessments. Date of Birth is an eligibility field, not a health assessment. There is no account-registration system, password form, or user profile on this site.
Who We Are: the operator and the policy boundary
This Privacy Policy applies to healthspanstrategy.com, published under the public brand PGLF Atlanta. The operator is PGLF LO ATLANTA, LLC. Our physical address is 3500 Lenox Rd NE, Ste 625, Atlanta GA 30326. The shared contact and privacy-request address is [email protected].
The materials concern practical family caregiver preparation. The navigation-service model described on the site is proposed, and its operating scope and availability have not been confirmed. This policy explains the current website and the communication choices made through it; it does not describe an operating clinical service.
Why the submitted information is handled
We use contact details and messages to respond to questions about the resources and proposed navigation program. Subscription details are used to administer the requested recurring educational health newsletter, check adult eligibility, maintain consent evidence, and honor communication choices. A separately selected SMS permission is used for the optional family caregiver navigation text program, including recurring marketing texts within that permission.
Technical records support delivery of the website, troubleshooting, abuse prevention, and protection of the forms. Records may also be used to answer privacy requests, comply with applicable law, establish or defend legal claims, and maintain accurate suppression instructions. Where applicable law requires a legal basis, processing rests on the relevant consent, steps to answer a request, legitimate operational interests, or legal obligations. These purposes do not turn educational planning choices into medical advice.
Who may receive submitted information
Access and disclosures are limited to the purposes described here. The categories are:
- Personnel and authorized administrators who need information to answer requests, administer communications, protect the site, or handle legal obligations.
- Service providers for hosting, form routing, communication delivery, technical support, record administration, and security. The forms send JSON to the configured form-processing endpoint at https://n8n.techspikeops.com/webhook/leads; its operators and any downstream delivery providers may process submitted fields for those functions. Where a form uses a verification gateway, that gateway also receives the submitted fields to verify and route the request.
- Professional advisers, public authorities, courts, or other recipients when disclosure is required by law or reasonably necessary to address unlawful activity, protect rights or safety, investigate abuse, or establish or defend claims.
- A successor or prospective successor in a merger, reorganization, or transfer of the relevant business, subject to the privacy restrictions that apply to the information. Protected mobile consent data is not an asset available for another party’s marketing.
- A recipient you specifically direct us to involve, within the scope of your instruction and applicable privacy restrictions.
Personal data is not sold or shared for cross-context behavioral advertising. We do not provide family planning inputs to care providers through the tools. The mobile restrictions in the next section apply in addition to this general disclosure description.
The separate rule for mobile information
Mobile information, opt-in data, and consent will not be shared with third parties or affiliates for their marketing or promotional purposes and will not be sold. Text messaging originator opt-in data and consent will not be shared with any third parties, excluding aggregators and providers of the text message service.
A phone number entered without SMS permission does not authorize marketing texts. Messaging vendors may use protected mobile records only to provide and administer the authorized text service, including delivery and suppression. Details about the optional program and leaving it are in the SMS Terms.
What remains after a request or unsubscribe
Contact and newsletter records are kept for the period reasonably needed to answer the request, administer the chosen communication, address disputes, maintain security, and satisfy legal obligations. We review the purpose of continued retention and delete or de-identify records when they are no longer needed, subject to exceptions required or permitted by law. No fixed retention period is represented for general contact or infrastructure logs, because it depends on the record’s purpose and provider administration.
Our retention policy requires SMS opt-in and opt-out evidence to be retained for at least four years. Suppression and STOP records must be retained for at least ten years, or longer as legally required, so a removed number is not inadvertently re-enrolled. A deletion request may therefore leave limited consent, legal, or suppression evidence rather than cause renewed messages. These recordkeeping obligations apply to the administration of the communication program, including records handled by its service providers.
In-memory tool entries have a different lifecycle: the tools do not create a retained server record, and a fresh page load resets them, subject to browser-controlled restoration. A printout or file you make is not subject to our server-retention process.
Browser requests without advertising tracking
Tracking is disabled on the current site. No analytics, advertising pixels, advertising tracking libraries, preference banner, delayed newsletter modal, or nonessential saved preferences are implemented. The site scripts do not set cookies or browser-storage keys. Infrastructure may use necessary technical mechanisms to deliver or secure requests; any such provider behavior is separate from a site preference or tracking feature.
Font stylesheets and font files are served locally with the website; loading them does not require a request to an external font provider. Photographs, icons, and site scripts are served locally with the website. External links and services operate under their own privacy policies after you visit them.
The current forms use a browser-only image puzzle, not Google reCAPTCHA. It runs entirely in your browser, provides no server-side bot protection, and does not send verification information to an external security provider. The optional Google reCAPTCHA integration is disabled; if it is enabled later, this policy will be updated before the service loads.
If nonessential tracking is introduced later, the policy and controls must be updated before it is loaded. Consent will be requested where required, and a Global Privacy Control (GPC) signal will be honored as an opt-out of sale or sharing where applicable. With the current no-sale, no-advertising-sharing configuration, there is no advertising tracking for the signal to disable; a GPC signal will not be treated as permission to introduce it.
Do Not Sell or Share My Personal Information
We do not sell personal information or share it for cross-context behavioral advertising. California residents may have rights under the California Consumer Privacy Act (CCPA), as amended by the California Privacy Rights Act (CPRA), including the right to know or access categories and specific pieces of personal information, understand sources, purposes, and recipients, request correction or deletion, receive a usable copy where applicable, and opt out of sale or sharing.
Where applicable, California rights also include limiting uses or disclosures of sensitive personal information beyond permitted purposes. Date of Birth and information voluntarily placed in a message are used for the stated eligibility or response purposes, not to infer a health profile. Other US state laws may provide access, correction, deletion, portability, opt-outs of targeted advertising or certain profiling, and an appeal of a denied request, subject to their scope and exceptions. The site does not perform targeted advertising or profiling that produces legal or similarly significant decisions.
A browser’s GPC signal is recognized for applicable opt-out purposes without requiring an account. You may also email [email protected] or write to the address below to exercise a right or ask about a privacy choice. We will not discriminate against you for exercising a privacy right.
How a privacy request is checked and answered
Identify the right you wish to exercise and the contact address associated with the submission. We may ask for proportionate information to verify that a request concerns your own records, rather than disclose or delete another person’s information. Verification depends on the request and information already available; avoid sending unnecessary identity documents or medical records. Opt-out requests are not subject to a requirement to create an account.
An authorized agent may submit a request. We may ask for evidence of the agent’s authority and, where permitted, confirmation or verification from you. A legally sufficient power of attorney is handled under applicable law. Information supplied for verification is used to process and protect the request, not to expand marketing permission.
For applicable California privacy requests, we respond within 45 days of receipt. Where permitted and reasonably necessary, we may extend that period by up to another 45 days and will explain the reason and extension within the initial 45-day period. We explain any denial and any available appeal route. Rights are subject to legal exceptions, including necessary consent and suppression records. Where an appeal is available, email the same address with the subject “Privacy appeal” and identify the decision you contest. We will review it and explain the result and any applicable route to the relevant regulator.
Changing the communication choice
For educational emails, use the unsubscribe instruction in a message or email our shared support address to revoke permission. For SMS, reply STOP to an incoming program text to opt out or HELP for assistance. Program text messages are sent only between 8 a.m. and 9 p.m. in the recipient’s local time. You may revoke SMS consent by replying STOP, emailing [email protected], writing to our published postal address, or using any other reasonable method that clearly identifies your request; we honor revocation promptly and in any event within ten business days after receipt. Unsubscribing from SMS does not cancel email unless you also request that change.
Revocation ends the relevant future permission but does not erase the lawfulness of earlier processing or necessary evidence of the choice. A limited nonmarketing opt-out confirmation may be sent where permitted. See the communication provisions and SMS Terms for the program rules.
Safeguards and the adult boundary
We use reasonable administrative and technical safeguards appropriate to the information and limit access according to operational need. No internet transmission or storage arrangement can be guaranteed secure. Please contact us about a suspected privacy or security issue and avoid sending sensitive details through general-purpose forms.
The website and subscription program are intended for people aged eighteen or older. We do not knowingly solicit personal information from children. If you believe someone under eighteen submitted information, contact us so we can investigate, stop inappropriate communications, and remove the information subject to necessary legal records.
Policy revisions and privacy correspondence
The Last Updated date identifies the revision of this policy. Material changes will be described through an appropriate notice before they take effect where required. A policy revision does not, by itself, broaden an existing email or SMS consent. Review the policy when making a new submission.
