Reading, printing, and agreeing to these terms
These Terms & Conditions govern use of healthspanstrategy.com and its PGLF Atlanta educational resources. By using the website, you agree to these terms to the extent permitted by applicable law. If you do not agree, stop using it. The Privacy Policy explains information handling, and the SMS Terms add rules for the separately selected text program.
You may read the guides, assemble a conversation agenda, compare general pathway questions, and print a family meeting sheet for lawful personal family planning. These tools organize questions; they do not determine which care arrangement is suitable. A contact submission does not book an appointment, create a professional engagement, or confirm availability of the proposed navigation service.
A question list is not a clinical plan
PGLF Atlanta provides general family-caregiver resources and separately offers an optional marketing SMS program about its proposed family caregiver navigation program. The resources are not individualized medical advice, diagnosis, or treatment, and using the website or enrolling in messages does not establish a doctor-patient relationship. Operating navigation services and launch availability have not been confirmed.
Before changing diet, exercise, or healthcare plans, consult a licensed physician or qualified healthcare professional who can consider the person’s circumstances. A caregiver checklist cannot take the place of that evaluation.
Adult participation and responsible use
The site and communication programs are for adults aged eighteen or older. Provide accurate information about yourself and submit a mobile number only if you are its subscriber or otherwise have authority to authorize the described texts. Do not provide another person’s details without the permissions needed for that submission.
Do not use the website to harass others, transmit unlawful or deceptive material, infringe rights, introduce malicious code, probe or disrupt systems, evade protections, send unsolicited communications, impersonate another person, or gather information through unauthorized automated means. Keep sensitive medical records out of contact messages. You remain responsible for your device, connection, and lawful use.
What a submitted message authorizes
A contact message authorizes us to use its contents as reasonably needed to receive, route, and respond to the request and administer related records under the Privacy Policy. It does not grant permission to publish your message or to treat its contents as marketing consent. You keep rights in material you own; submit only material you have the right to share.
Planning-tool notes remain in the browser’s memory and are not submitted to us by those tools. You are responsible for safeguarding a printout or file you make and for respecting your relative’s preferences about what family members may see. We do not review tool entries or monitor the site as an urgent-care channel.
The separate communication agreement under the TCPA
The Telephone Consumer Protection Act (TCPA) and applicable rules govern certain automated calls and text communications. The required email checkbox requests the recurring automated educational health newsletter, Caregiver Notes. Optional SMS authorization is a separate, prior express written consent: checking it and submitting the form authorizes PGLF Atlanta to send recurring automated marketing text messages about the family caregiver navigation program to the number supplied, using automated technology.
SMS consent is not a condition of purchase or of the educational email subscription. The phone field may be blank if SMS is not selected. Message frequency varies, and message and data rates may apply under your mobile plan. The checkbox text, choice, date/version, page URL, and other consent records establish the scope of the submitted request; no general website visit authorizes marketing texts.
You may revoke SMS permission by replying STOP, ask for assistance by replying HELP, or contact [email protected]. Another reasonable method clearly expressing a wish to stop messages is also accepted; STOP is not the exclusive means of revocation. We honor revocation promptly and in any event within ten business days after receipt. A permitted nonmarketing confirmation of opt-out may follow. Email permission can be withdrawn through the unsubscribe instruction in an email or by contacting the same address. Changing one channel does not automatically change the other.
Delivery depends on network and carrier availability. T-Mobile is not liable for delayed or undelivered messages. Carriers are not liable for delayed or undelivered messages. Do not rely on the text program for time-sensitive or medical information.
The standalone mobile privacy clause and SMS privacy terms govern mobile information and consent evidence. Mobile information, opt-in data, and consent will not be sold or shared with third parties or affiliates for their marketing or promotional purposes. Text messaging originator opt-in data and consent will not be shared with third parties except aggregators and providers of the text message service.
Resource ownership and destinations beyond the site
Website text, designs, marks, and other materials belong to the operator or their respective rights holders. The permission to read and print for personal use is not permission to republish the resource library, resell materials, remove ownership notices, or imply endorsement. Uses beyond a right granted here or by applicable law require permission from the relevant rights holder.
Third-party links and external services may have their own terms and practices. A link is provided for navigation, not a promise of accuracy, suitability, safety, availability, or endorsement. The website does not control external content, carrier networks, or your browser’s printing functions.
Availability, uncertainty, and limits of liability
The website and its materials are provided “as is” and “as available,” to the fullest extent permitted by law. We disclaim warranties, whether express, implied, or statutory, including merchantability, fitness for a particular purpose, and noninfringement to the extent those disclaimers are permitted. We do not warrant that content is complete or current, that tools will be error-free, that access will be uninterrupted, or that a question list, subscription, or family conversation will produce an outcome.
To the fullest extent permitted by law, the operator is not liable for indirect, incidental, special, consequential, exemplary, or punitive loss arising from website use, reliance on educational material, interruption, or third-party services, including lost data or lost opportunities. We do not exclude liability that cannot lawfully be excluded, including responsibility for fraud, willful misconduct, or other matters protected by applicable law. Mandatory consumer protections and remedies remain available. Individual circumstances vary, and you must obtain appropriate professional input for decisions about care.
Responsibility for claims caused by misuse
To the extent permitted by law, you agree to indemnify the operator against reasonable third-party claims, losses, and expenses caused by your unlawful use, infringement of another person’s rights, or a material breach of these terms. This obligation does not cover claims caused by the operator’s own unlawful conduct or impose liability beyond what applicable law allows. We will give reasonable notice of a covered claim and an opportunity to participate in its response; a settlement may not impose a new obligation on you without your consent.
Georgia law and a practical dispute path
These terms are governed by the laws of the State of Georgia, United States, without applying choice-of-law rules that would displace mandatory protections. Applicable federal law and any nonwaivable consumer rights remain controlling where required.
First send a written description of a concern to our contact address, including the affected resource or communication and the resolution sought. The parties should have a reasonable opportunity to discuss a resolution. If the dispute remains unresolved, it may be brought in a court with competent jurisdiction in Georgia, except where applicable law permits or requires another venue. These terms do not require binding arbitration, waive a right to a small-claims proceeding, or prevent a complaint to a regulator.
Changes, interrupted access, and surviving provisions
We may revise the site or these terms and may restrict access for misuse, security concerns, or discontinuation. The Last Updated date identifies this revision. Material changes will be communicated where required; a new version does not retroactively expand communication consent. Discontinuing a program does not remove the duty to respect existing suppression records.
If a provision is unenforceable, the remaining terms continue to apply to the extent permitted by law. Provisions that by their nature must survive, including lawful recordkeeping, ownership, and dispute provisions, remain applicable. These terms and the incorporated program terms describe this website agreement and do not create an operating care-service contract. Rights that cannot be waived are preserved.
